Remote Heart Failure RN - MA licensed
We are seeking a dedicated Remote RN to provide high-quality, virtual heart failure care for patients enrolled in our platform. The RN will play a critical role in delivering remote patient management, facilitating self-management and education, and ensuring seamless care coordination for patients in partnership with our provider(s) and local provider teams. This role offers the unique opportunity to work remotely while engaging directly with heart failure patients, optimizing their outcomes through advanced, tailored care.
Key Responsibilities:
- Patient Monitoring & Virtual Care Management: Utilize our platform to monitor patient data and trends, respond to alerts, and engage with patients for real-time, personalized interventions.
- Care Coordination: Collaborate with our clinical team to coordinate and deliver virtual care, ensuring high standards and continuity.
- Patient Education & Self-Management Support: Educate patients on managing heart failure, medication adherence, and lifestyle changes. Provide support to empower patients in following their care plans.
- Clinical Documentation: Maintain thorough documentation of patient interactions, clinical notes, and follow-ups in alignment with provider preference and regulatory / platform requirements.
- Alert Management & Triage: Respond promptly to clinical alerts and patient messages, providing timely advice according to established protocols.
- Data Review & Reporting: Regularly review RPM and patient-reported data, identifying trends and reporting any abnormal findings to the care team as needed.
Qualifications:
- Licensure: A Registered Nurse (RN) licensed to practice in Massachusetts is required.
- Experience: 3+ years of outpatient clinical experience, preferably in cardiology, but recent graduates interested are welcome to apply.
Skills:
- Strong ability to deliver virtual patient care and education.
- Proficiency with digital health tools, clinical documentation, and patient data management.
- Effective communication skills for patient interaction and care coordination.
Preferred Qualifications:
- Bachelor’s Degree in Nursing (BSN) preferred.
- Experience with remote patient monitoring (RPM) and virtual health platforms.
- Heart Failure experience and/or familiarity with heart failure treatment protocols and guideline-directed medical therapy (GDMT).
