The Patient Monitoring Care Assistant tracks Biometric data and sends out-of-range data to the appointed qualified professional employee in the practice.   

Under the direct supervision of a Registered Nurse (RN), Nurse Practitioner (NP), Social Worker (SW/MSW), or Physician (MD). Gathers and documents patients’ data in an electronic system, and communicates data to RN, NP, or MDs using technologies such as the Internet, computers, telephones, digital assessment tools, and telemonitoring equipment. Assist patients in setting up troubleshooting monitoring devices.  

Primary Responsibilities:

  • Daily monitoring of ordered and defined biometric parameters.
  • Notifies appointed qualified professionals when biometric parameters are out of range.
  • Set up and train patents on monitoring devices.
  • Makes follow-up phone calls to patients as necessary to resolve technical items.  
  • Comply with all HIPAA guidelines, maintaining the confidentiality of clients and patients.
  • Comply with Federal and respective state laws regulating health management organizations and telephone information centers and all department standards policies and procedures Licenses/Certifications.
  • Perform duties ONLY within the scope of practice approved by the state in which the patient is located. · Provides utilization management and quality improvement in accordance with utilization policies and processes. Ensures adherence to the Quality Management Program.  
  • Performs other job-related duties as assigned.

Required Qualifications:

  • Certified Medical Assistant in Texas
  • Knowledge of legal and ethical issues related to patients’ rights, rehabilitation terminology, discharge guidelines, community support services, and funding.
  • Strong interpersonal, communication, and teamwork skills.
  • Proficient in English and Spanish.

Preferred Qualifications:

  • Understanding of management practices or health plan administration of referral process, triage, or medical management.
