We are seeking a highly skilled Clinical Nurse Manager - Connected Care Program to lead and coordinate the activities of our care and case management team, ensuring the delivery of high-quality patient care and effective care coordination. In this role, you will drive team performance to achieve specific targets, ensuring that outcomes for our members meet the highest standards. As a key collaborator, you will be instrumental in implementing new operational workflows, services, and expectations across the organization. You will also play a vital role in fostering professional growth and development within your team.
Responsibilities:
- Manage the daily operations of the care management team.
- Hold regular team meetings and performance reviews.
- Ensure care management is focused on the member and follows best practices.
- Ensure compliance with company policies and regulatory standards.
- Guide and support staff with complex cases and phone interactions.
- Find areas for improvement and lead quality initiatives.
- Act as a liaison between care & case management and other stakeholders.
- Set and manage individual team member performance goals, including volume, customer commitments, service line agreements, and performance targets.
- Help with daily tasks and patient care as needed.
- Attend biannual management off-sites (dates/locations to be determined).
Qualifications:
- Registered Nurse (RN) in good standing with the nursing board of your state.
- Compact state license required, with a willingness to obtain licensure in non-compact states.
- Bachelor’s degree in nursing or related field.
- At least 5 years of remote case management experience.
- Minimum 2 years of leadership experience in case management, with preference for experience managing remote teams.
- Excellent organizational, team-building, and attention to detail skills.
- Proven experience in leading teams and initiatives with measurable results.
- Technology proficiency required, including data-driven decision-making.
- Strong ability to manage change and a proven track record of learning agility.
