A rapidly growing nationwide healthcare organization is offering FULL TIME WORK for medical assistants. As an MA you'll work under physician general supervision. You'll employ great teamwork and communication with a team of Nurses and Medical Assistants. The company - an early leader in telehealth - engages geriatric patients who face multiple chronic conditions and geographical barriers to care. As a Command Center MA, you'll work to assess and evaluate patient needs to achieve quality service. You'll have the opportunity to employ a variety of care management strategies and processes. By offering telephone and video visits, you can assess and support patient health, offer education, identify resources, and help remove barriers to achieving health and independence. Job Description: *Provide appropriate medical device support including installation, troubleshooting, replacements and adherence coaching. *Evaluate user needs and requirements to achieve and/or maintain optimal wellness state using the digital health platform *Assess Medicare, Medicaid, and Health plan members holistically over phone and video *Guide users and their caregivers toward, and facilitate interaction with, resources appropriate for user wellbeing *Participate as a multidisciplinary team member and provide direction to other care command center team members to maintain and improve user health. *Notify care team lead or nurse of changes in patient condition or status. *Demonstrate a professional attitude, sound judgment and continuity in interactions with physicians, other health care providers, and community agencies. Requirements: *Bilingual proficiency - English and Spanish - very strongly preferred *Remote patient monitoring, telehealth, chronic condition management, and individualized care planning experience are a plus. *Certified Medical Assistant, or training, from an accredited school. *Must possess telephone, typing, and virtual communication prowess *Able to work 40 hours per week between the hours of 7am - 5pm PT with occasional Saturdays.
