DirectShifts seeks a dynamic, team-oriented Family Nurse Practitioner (FNP) who is skilled in the provision of primary / urgent care for the managed Medicaid population. The ideal candidate will function independently in a collaborative environment. 

Responsibilities:

  • Confidence and comfort in assessing the patient initial evaluation, history, and physical examination of patients whether they are primary care or urgent care patients
  • Make decisions regarding data gathering and appropriate management and treatment for the initial evaluation of a problem or the follow-up of a previously diagnosed condition
  • Provides patient care management within advance practice parameters; manage acute care and chronic care activities
  • Assist team members with inbound clinical triage
  • Identify, develop, and implement clinical protocols for patient treatment
  • Assist with patient education activities
  • Conduct presentations and community sessions pertaining to disease specific process and wellness
  • Monitor the quality of care delivered to our members
  • Document patient contacts appropriately; complete chart reviews on a regular basis and in a timely manner
  • Provide and receive feedback on quality of care being delivered regulatory standards are met

Requirements:

  • Graduate of an accredited program for Family Nurse Practitioners (FNP)
  • Current licensure as a Nurse Practitioner in state of Pennsylvania
  • BLS Certification
  • Current Certification by the American Academy of Nurse Practitioners or American Nurse Credentialing Center
  • Meets all requirements to obtain associate medical staff membership and required privileges of service.
  • Eligible for a DEA and NPI
  • Experience with EMR records preferable
  • At least 1-2 years of experience in primary care, preferably in a family practice setting that treats pediatric patients
  • Experience in an urgent care environment a plus
