DirectShifts is looking for a Full-time Social Worker in Hampstead, NH.
Social Worker performs case management services and actively participate in the Treatment Planning process with the multidisciplinary team.
This position provides individual, couples, family, and group therapy. Responsible for developing a psychosocial formulation and completing the Social Work Assessment.
Social Workers are tasked with conducting level-of-care assessments and related insurance reviews for admissions.
All aspects of aftercare planning are the responsibility of the assigned social worker.

Key Job Responsibilities
Provide direct and indirect social work services to patients and their families, to include: group and individual counseling, case management, psychosocial assessments, aftercare planning, and facilitating family meetings.
Coordinate discharge planning and continuing care plan in accordance with treatment team recommendations.
Provide clinical consultation to other treatment team members.
Develop and maintain professional relationships with other community healthcare providers. Provide consultation to collateral outpatient providers, residential programs, and schools as needed.
Perform level-of-care assessments and related pre-certification reviews with insurance companies as needed.
If assigned to admissions department, will complete level-of-care assessments, as well as assist with more administrative tasks to include: conducting phone intakes; facilitating all aspects of the referral process; and collaborating with medical staff and program directors regarding potential admissions.
Complete documentation in accordance with all Joint Commission, State Licensing, and Medicare standards applicable to Social Work.
LICSW’s may provide direct clinical supervision to less experienced social workers.
Promote the hospital culture of patient safety in a non-punitive atmosphere; report medical errors and significant events; participate in hospital safety training programs and Performance Improvement activities.
Master’s degree in Social Work from an accredited graduate program.
Related field work experience and/or previous experience with patients/families in a psychiatric setting.
Eligible Social Workers are encouraged to complete LICSW certification process.
CPR Certification (within 3 months of hire)
CPI Certification (within 3 months of hire)
