Job Details:

  • Shift: Day Shift: 7:00 am - 3:00 pm, Evening Shift: 3:00 pm - 11:00 pm; Every other weekend required
  • Employment Type: Contract

Responsibilities:

  • Assess, plan, evaluate, and implement patient care according to standards of practice
  • Document patient care delivery in medical records
  • Administer medications and treatments, observing patient responses
  • Establish and maintain positive relationships with residents, families, and staff
  • Provide patient and family education on health practices and treatment modalities
  • Conduct resident care rounds, receive shift reports, and distribute assignments
  • Initiate assessments of residents’ physical, psychological, social, and rehabilitative needs
  • Participate in Comprehensive Care Planning and Minimum Data Set Plus processes
  • Supervise and ensure care adheres to facility policies and procedures
  • Assist with mealtime assessment and feeding of residents
  • Provide orientation, training, and evaluation for new nursing personnel
  • Participate in departmental and interdisciplinary committees influencing nursing practice
  • Maintain professional competence through continuing education and performance improvement activities
  • Perform emergency nursing measures as necessary

Requirements & Qualifications:

  • Experience in Cardiac/Angio/Neuro/Vascular & Trauma cases
  • Proficiency in delivering nursing interventions for physical and behavioral conditions
  • Ability to communicate effectively and document nursing interventions and treatments
  • Familiarity with Fire and Safety regulations and maintaining a safe environment
  • CPR Certification required
  • PRI Assessor certification preferred
  • New York State Registered Nurse License required
