Job Title: Medical-Surgical Registered Nurse (Med-Surg RN)

Location: Port Jefferson, NY

Shift : Days / Nights

Position Summary

The Med-Surg RN provides direct patient care to adult patients with a wide range of medical and surgical conditions. Responsibilities include assessing patient needs, developing and implementing care plans, administering medications and treatments, monitoring patient progress, completing OASIS documentation when applicable, and collaborating with the interdisciplinary healthcare team to achieve high-quality patient outcomes.

Key Responsibilities

  • Assess, plan, implement, and evaluate patient care plans.
  • Monitor patient condition and report changes appropriately.
  • Administer medications, IV therapy, and treatments safely.
  • Provide pre- and post-operative nursing care.
  • Complete OASIS assessments and maintain timely, compliant clinical documentation as required.
  • Maintain accurate and timely documentation in the EMR.
  • Educate patients and families regarding treatment plans and discharge needs.
  • Collaborate with physicians, therapists, case managers, and ancillary staff to optimize patient outcomes and functional recovery.
  • Monitor patient responses to treatment and communicate significant changes to the care team.
  • Ensure compliance with hospital policies, patient safety, and infection control standards.
  • Respond effectively in emergency situations.
  • Maintain a safe and therapeutic environment for patients.

Qualifications

  • Active New York State RN License required.
  • BLS certification required.
  • ACLS certification preferred.
  • BSN preferred.
  • Recent RN experience in Med-Surg, Rehabilitation, Home Health, or related acute care settings preferred.
  • Proficiency with OASIS assessments and documentation preferred.
  • Experience caring for patients with chemical dependency and/or substance use disorders preferred.
  • Strong assessment, communication, critical-thinking, organizational, and care coordination skills.
