Job Description:

OneTouchMD is seeking an experienced Nurse Practitioner with multi-state licensure (preferably all 50 states) to join our growing nationwide telehealth program focused on Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) for patients with diabetes, hypertension, obesity, and metabolic disorders.

You’ll work closely with a supervising physician and interdisciplinary care team, coordinating patient outreach, data review, and telephonic/virtual patient engagement through our digital health platform.

Key Responsibilities:

  • Review remote monitoring data (vital signs, glucose, blood pressure, weight, etc.) and identify abnormal trends.
  • Conduct monthly CCM calls and patient check-ins per CMS billing requirements.
  • Provide health coaching and patient education regarding medication adherence, nutrition, and lifestyle goals.
  • Document all encounters and communications in EHR per compliance standards.
  • Escalate clinical concerns to the supervising physician as needed.
  • Coordinate care between patients, dietitians, and other clinical staff.
  • Ability to prescribe medications within state scope of practice.
  • Participate in case reviews and ongoing program improvement initiatives.

Qualifications:

  • Active NP license in multiple states (multi-state or compact license required; all 50 states preferred).
  • Minimum 1 year of experience with Remote Patient Monitoring (RPM) and/or Chronic Care Management (CCM).
  • Working knowledge of CMS CPT codes (99453, 99454, 99457, 99458, 99490, 99491).
  • Prior experience using telehealth or virtual care platforms.
  • Strong clinical judgment, communication, and follow-up skills.
  • Must have secure internet access and a quiet, professional telehealth workspace.

Preferred Experience:

  • Familiarity with platforms such as CoachCare, HealthSnap, Prevounce, or SmartCare.
  • Experience with diabetes education, hypertension management, or obesity programs.
  • Experience collaborating with dietitians, care coordinators, or AI-based triage tools.
  • Compact license (NLC) or active licensure in multiple jurisdictions.
