Job Description:
We are seeking an experienced Registered Dietitian / Nutritionist who is passionate about chronic disease prevention, medical nutrition therapy (MNT), and long-term patient coaching. You will play a central role in delivering evidence-based diabetes management, obesity/weight-management support, and lifestyle coaching within our virtual care platform.

You will work closely with our physicians, nurse practitioners, care coordinators, and remote monitoring team to provide personalized nutrition care plans, ongoing telehealth coaching, and structured follow-ups tailored to each patient’s clinical needs.

This position is ideal for a Licensed Dietitian who excels in virtual patient engagement, motivational interviewing, and structured MNT protocols.

Key Responsibilities:

1. Medical Nutrition Therapy (MNT)

    Deliver comprehensive MNT to patients with:

  • Type 2 diabetes
  • Prediabetes
  • Metabolic syndrome
  • Obesity & weight management needs
  • Hyperlipidemia, hypertension, and related metabolic conditions
  • Patients using GLP-1 therapies (semaglutide, tirzepatide, liraglutide, etc.)

MNT Duties Include:

  • Conduct initial nutritional assessments via video or phone.
  • Develop individualized, clinically appropriate MNT care plans.
  • Adjust nutrition interventions according to patient progress, labs, provider input, and RPM data (glucose, weight, BP, etc.).
  • Provide evidence-based education on blood sugar stabilization, GLP-1-related nutritional needs, appetite regulation, and GI symptom management.
  • Document all encounters according to clinical and regulatory standards.

2. Coaching, Follow-Up & Patient Engagement

Provide regular coaching sessions based on the patient’s plan tier (monthly, bi-weekly, weekly).

You will:

  • Conduct motivational interviewing and behavior-change coaching.
  • Provide structured follow-up through calls, secure messaging, and asynchronous check-ins.
  • Guide patients through meal planning, macronutrient literacy, portion control, and habit-building.
  • Help patients overcome barriers related to nutrition, medication adherence, and lifestyle change.
  • Track and monitor patient progress, including weight, symptoms, medication response, and RPM data.

3. Care Coordination and Clinical Team Collaboration

  • Work closely with program physicians and NPs to support medical treatment plans.
  • Provide clinical insights during patient case reviews, including nutrition adjustments for GLP-1 dose changes.
  • Communicate changes in patient status or concerns to medical staff.
  • Collaborate with health coaches, billing staff, and care coordinators as needed.

4. Documentation & Compliance

  • Maintain accurate clinical records within the electronic health platform.
  • Ensure documentation meets standards for MNT, telehealth, insurance billing, and compliance.
  • Follow HIPAA and regulatory guidelines for telehealth care.
  • Provide patient notes, follow-up summaries, and MNT reports as required.

Qualifications Required

  • Registered Dietitian Nutritionist (RDN) credential from CDR.
  • Active state licensure in multiple states (all 50 states highly preferred but not mandatory).
  • At least 2+ years clinical experience in one or more of the following:
  • Diabetes education
  • Weight management
  • Obesity medicine
  • GLP-1 patient support
  • Chronic disease management
  • Experience delivering MNT via telehealth.
  • Strong skills in motivational interviewing, coaching, and patient communication.
  • Ability to work independently and manage a virtual case load.

Preferred

  • Experience with RPM-supported programs (glucometers, heart rate monitoring, weight scales).
  • Familiarity with GLP-1 therapies (Wegovy, Ozempic, Zepbound, Victoza, Saxenda, etc.).
  • Experience in a digital health, telehealth, or virtual coaching program.
  • Certified Diabetes Care and Education Specialist (CDCES) is a plus.
  • Multilingual capability is a plus.
