Hiring Nurse Practitioners – In-Home Preventative Care | PA
We are seeking a compassionate and dedicated Nurse Practitioner (NP) to join our mission-driven team, delivering high-quality, patient-centered care directly to homes across the Greater Philadelphia area. This is a unique opportunity to make a meaningful impact by providing preventative and chronic care management to underserved communities—especially older adults who benefit most from in-home services.
Key Responsibilities:
- Conduct comprehensive in-home health assessments and provide preventative care services
- Develop individualized care plans for patients with chronic conditions
- Perform health screenings, with a focus on geriatric and vulnerable populations
- Support wound care as part of a broader treatment plan (preferred)
- Educate patients and their families on disease prevention, health maintenance, and wellness strategies
- Maintain accurate, up-to-date clinical documentation in accordance with healthcare regulations
Qualifications:
- Active NP certification
- Experience in home-based or geriatric care (preferred)
- Wound care experience is a plus
- Great communication and rapport-building skills
- Reliable transportation required
