Home Health Field Nurse- Hospice, Palliative Care | Brooklyn, NY

Responsibilities:
- Provide comprehensive nursing care to hospice patients in their homes across the New York boroughs
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs
- Develop and implement individualized care plans in collaboration with an interdisciplinary team
- Administer medications, treatments, and other interventions as prescribed
- Educate patients, families, and caregivers on disease processes, symptom management, and end-of-life care
- Ensure accurate and timely documentation of patient assessments, care plans, interventions, and outcomes
- Coordinate care with physicians, social workers, chaplains, and other healthcare professionals to provide holistic care
Requirements:
- Valid Registered Nurse (RN) license in the state of New York
- Minimum of 1 year of clinical experience in hospice, palliative care, or home health nursing
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Compassionate and empathetic approach to patient care
- Willingness to travel throughout the New York boroughs to visit patients in their homes

Salary: $55.00 - $70.00 per hour + Full Benefits + Sign-On Bonus!
