About the Role:
As a Home Health RN at DirectShifts, you will play a vital role in providing comprehensive care to homebound patients across California . You will work independently, collaborating with physicians and other healthcare professionals, to develop and implement individualized care plans that meet the unique needs of each patient in their home environment.
Responsibilities:
- Conduct patient assessments, monitor vital signs, and manage symptoms according to physician orders.
- Administer medications, perform wound care, and provide skilled nursing interventions like injections or catheter management.
- Educate patients and their families on disease management, self-care strategies, and medication administration.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated care.
- Provide emotional support and guidance to patients and families facing illness or recovery at home.
- Document patient care comprehensively and maintain accurate medical records.
- Ensure a safe and sanitary home environment for patients.
- Manage your schedule effectively and prioritize patient needs in a fast-paced environment.
Qualifications:
- Active and unrestricted California State RN License.
- Minimum 1 year of experience in a medical/surgical setting (experience in home health care is preferred).
- Strong understanding of adult health conditions, common procedures, and medication administration.
- Excellent communication, critical thinking, and problem-solving skills with the ability to work independently and make sound clinical judgments.
- Compassionate and patient-centered approach with a focus on building trust and rapport with patients and families.
- Ability to manage time effectively, prioritize tasks, and adapt to the unique demands of the home healthcare setting.
- Valid driver's license and reliable transportation are required.
