Position Overview
We are seeking a Hospital Pharmacist to join the pharmacy team at Interfaith Hospital. Under the general supervision of the Director of Pharmacy and/or Pharmacy Supervisor, the Pharmacist will safely and accurately prepare and dispense medications while ensuring compliance with hospital policies and New York State and federal regulations.
Key Responsibilities
- Review, interpret, calculate, prepare, and dispense medications based on physician orders.
- Identify and resolve unclear, incomplete, or questionable medication orders in collaboration with prescribers and nursing staff.
- Review medication profiles for allergies, drug/food interactions, therapeutic duplication, and other potential issues.
- Recommend appropriate changes to drug therapy when needed.
- Prepare labels and dispense medications throughout the hospital.
- Enter medication orders accurately into the pharmacy computer system.
- Provide medication information, including therapeutic effects, interactions, and potential adverse reactions, to physicians, nurses, and patients.
- Compound and label IV admixtures, chemotherapy, and parenteral nutrition solutions according to established procedures.
- Supervise and verify the work of Pharmacy Technicians and Aides in sterile and non-sterile areas.
- Dispense controlled substances in accordance with applicable regulations.
- Participate in staff orientation, training, education, medication-area inspections, and quality improvement activities.
- Demonstrate appropriate knowledge of age-specific medication dosing for neonatal, pediatric, adult, and geriatric patients.
- Work across pharmacy service areas and perform other duties necessary to support safe, high-quality patient care.
Requirements
Education
Bachelor's degree in Pharmacy required.
Experience
Minimum 1 year of hospital pharmacy experience required.
License
Active New York State Pharmacist license and current registration required.
Skills
- Strong verbal and written English communication skills.
- Computer and word-processing proficiency.
- Ability to work effectively in a hospital pharmacy environment.
Physical Requirements
