Our client partners with physician groups and home care agencies to help implement and deliver these services; providing administrative support, and helping to find committed and capable staff for the physician group.
What you will do :
- Receive clinical calls and triage
- Utilize telehealth system and perform virtual visits
- Coordinate care appropriately and timely with members of the care team both internal and external
- Have the ability to educate members, family or other caregivers on chronic conditions, diet changes, and pieces of their care plan
- Have confidence in the ability to triage appropriately and escalate to appropriate resources
- Utilize technology for documentation
Minimum Qualifications :
- Fluency in English and Spanish in writing, reading, and speaking (required)
- Graduated from an accredited nursing program (required)
- Current RN License in good standing in the state of NY (required)
- At least 2 years of nursing experience providing care to adult and geriatric patient populations (required)
- Confidence with clinical skills in the performance of telephonic triage/assessment (required)
- The ability to work remotely and has a private area in their home/workspace (required)
- A genuine, compassionate desire to serve others and help those in need
- High-speed home WiFi/data connection to support company-provided IT equipment
