Remote Licensed Therapist | Collaborative Care Model
We are seeking a dedicated and experienced licensed behavioral health clinician to join our virtual care team and collaborative care model.This remote position allows you to play a crucial role in supporting the unique needs of women, starting with mental and behavioral health.
Location: Remote; must have license in either CT, GA, NY, NV, AZ, or TX
Schedule: 20 - 40 hours/week.
Responsibilities:
- Screen and assess patients for common mental health and substance abuse disorders using PHQ9, GAD7, or other tools as required and provide basic education and available treatment options.
- Use brief solution-focused and problem-solving check-ins typically 30 by video, augmented by asynchronous messages, voicemails, and short phone calls. Our patients can have mild to moderate anxiety and/or depression.
- Document patient engagement and follow-up care in our custom EHR. Track patient follow-up and clinical outcomes.
- Document patient progress and treatment recommendations in a registry so they can be easily shared with OBGYN/PCPs, psychiatric consultants, and other treating providers. Utilize the registry to identify and re-engage patients.
- Facilitate referrals for clinically indicated services outside the OBGYN or primary care clinic such as behavioral health specialty care, substance abuse treatment, etc.
Qualifications:
- Licensed Mental Health Counselor/Professional Counselor, Licensed Marriage and Family Therapist, Licensed Social Worker, or Licensed Psychologist.
- Minimum of 2 Years of experience with mental health/behavioral health counseling.
- License to practice either under supervision or independently
- Understanding and appreciation for integrated care
- Fluent in English, Spanish a plus.
