Our client MiraMace is a healthcare advocacy marketplace that connects patients and families with expert advocates who help them understand and take control of their personal health.
About the Role
As an RN Healthcare Advocate, you will support Medicare patients throughout their healthcare journey. You will guide patients through complex health concerns, empower them to achieve their wellness goals, and address Social Determinants of Health (SDOH). In this role, you’ll serve as both an empathetic listener and an action-oriented problem solver who ensures patients receive the care and resources they need.
Responsibilities
- Learn and adapt to clients systems, tools, technology, and partner workflows, while applying your clinical expertise in every patient interaction.
- Build strong, trusting relationships with Medicare patients, rooted in empathy and active listening.
- Identify, prioritize, and address patients’ healthcare needs to ensure continuity of care.
- Develop and implement holistic care plans that incorporate social determinants of health, such as food resources, transportation, and home support.
- Collaborate with patients and partners to streamline care and reduce barriers.
- Contribute to building innovative systems that enhance patient outcomes for Medicare populations.
Qualifications
- Active RN license in good standing.
- Strong knowledge of Social Determinants of Health and experience serving diverse patient populations.
- Demonstrated empathy and commitment to advocating for patients, especially those most at risk.
- Proven clinical expertise with exceptional organizational and communication skills.
- Ability to manage multiple priorities and work effectively under pressure while maintaining accuracy and clarity.
- Technologically proficient; comfortable learning and navigating new platforms and tools.
