We are currently hiring RN Case Managers for our Royal Oak, MI, office and are open to both full-time and part-time candidates.
Position Details:
- Job Title: RN Case Manager (Telehealth)
- Location: Royal Oak, MI (Onsite)
- Employment Type: Full-Time or Part-Time
- Full-Time Schedule: Monday–Friday, 8:00 AM–4:00 PM
- Part-Time Schedule: Flexible based on availability and operational needs.
- Work Environment: All patient care is provided virtually from our Royal Oak office. This is not a remote or travel nursing position.
Key Responsibilities:
- Conduct patient assessments and develop individualized care plans.
- Provide virtual patient education, follow-ups, and chronic care management.
- Monitor patient progress and coordinate care with healthcare providers.
- Document patient interactions and care plans accurately in our EHR system.
- Identify and escalate patient concerns as appropriate.
- Collaborate with our interdisciplinary clinical team to ensure continuity of care.
Required Qualifications:
- Active, unrestricted Michigan RN license.
- Current BLS certification.
- Minimum of 2 years of clinical nursing experience.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work with electronic health records (EHR) and telehealth platforms.
- Strong patient education and care coordination skills.
Preferred Qualifications:
- Previous experience in case management, chronic care management, or telehealth.
- Certified Case Manager (CCM) certification.
- Experience with remote patient monitoring (RPM).
