DirectShifts is looking for a Full-time Social Worker (BSW) in Lincoln, NE.

Summary

  • Responsible for collaborating with the Provisionally Licensed Clinical Social Workers (PLCSW), Licensed Clinical Social Workers (LCSW) and Licensed Independent Clinical Social Workers (LICSW) in completing assessments that include but are not limited to admission, continuing care needs, post hospitalization, recovery plans, and discharge plans.
  • Participates in planning with patients, their families and other members of the health care team including outside providers, payer sources and community, state and federal resources.
  • Documents all planning and coordination in the medical record. Facilitates psycho-educational groups and prepares for treatment team meetings.

Principal Job Functions

  • Coordinates daily work with supervisor and/or co-assigned staff member.
  • Interviews and meets with patients to obtain their input regarding admission, progress toward goals on their care plan and discharge plans.
  • Documents patient information and discharge plans according to departmental procedure and standards.
  • Collects clinical data of current health and psychological needs.
  • Provides input into the development of an interdisciplinary plan of care to meet identified needs of the patient and family.
  • Facilitates psycho-educational group meetings as directed.
  • Utilizes Trauma Informed Care Principles.
  • Engages in communication with patients from a variety of developmental, social and cultural backgrounds about current treatment, discharge needs and treatment recommendations.
  • Identifies appropriate service and funding resources.
  • Maintains a close working relationship with agencies and facilities that provide care to our patients in the community.
  • Performs all duties within legal limitations as properly authorized and delegated or assigned.
  • Completes continuing education requirements necessary to maintain certification/license in the State of Nebraska.
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Participates in meetings, committees and department projects as assigned.

Education And Experience

  • Bachelor’s degree in social work.
  • Current license as a Certified Social Worker in the State of Nebraska required.
  • Must be working toward a Master in Social Work
  • Must complete Master’s program within three (3) years
  • Some social work experience is preferred.
  • Experience with Mental Health population preferred.
