Job Duties:

Provides direct nursing care to assigned residents with complex needs as related to:

  • Hygiene and physical comfort
  • Medical treatment
  • Nutritional needs
  • Ambulation and exercise
  • Elimination Maintenance of residents’ safety
  • Maintenance of residents’ Safety

2. Directs the care of the residents performed by Certified Nursing Assistants.

3. Administers medication to assigned residents with exception of the following:

  • Blood and blood products
  • Hanging the first IV solution containing medications
  • Administering IV (push) medication
  • Administering IV medications via central Lines and Maintenance

4. Administers IV therapy with exception mentioned in (3) above:

  • LPN’s certified in IV therapy by Schulman and Schachne may perform venipuncture for the purpose of placing a heparin lock or administrating IV fluids
  • Set up and assist in starting IV fluids.
  • Hanging fluids and monitoring fluid rate as presented by physician order.
  • Replacing bags of IV fluids
  • Changing intravenous dressing, tubing on peripheral lines.
  • Discontinue IV fluids.

5. Administers treatment according to facility policy and procedure.

6. Prepares residents for diagnostic tests, operative procedures, medical regimen examinations and transportation.

7. Admits residents according to facility and unit policies and procedures and assists them in adjusting to the environment.

8. Participates in the Person Centered Care planning and coordinating of the nursing care in an assigned unit and implements plan.

9. Assists and/or contributes to the development and updating of the Person Centered Care Plan to reflect the residents’ needs.

10. Assists RN with health teaching for residents and families as needed.

11. Establishes positive relationship with residents and families in accordance with Residents’ Bill of Rights.

12. Participates in unit/team ICP meetings and PI teams.

13. Observes nutritional need of residents’. Actively participates at mealtime by assigning staff, feeding residents, monitoring intake of residents consumption.

14. Assist in post-mortem care of deceased residents’ as needed."

Skills:

  • Assist in maintaining a clean safe environment for residents and co-workers
  • Documents nursing care as per facility policies, in the Epic system Electronic Medical Records. Informs registered nurse and physician of residents’ problems and change in condition immediately
  • Assist registered nurse and physician in times of emergencies, including CPR as directed
  • Maintains competency in practice through participation in continuing education and other appropriate learning experiences
  • Participates in reporting to an oncoming nurse at change of shift
  • Respects the rights of residents’/patients and families
  • Assumes other-related duties as delegated by supervisory staff
  • Performs other related duties assigned by Registered Nurse. In an emergency performs other duties necessary for the welfare of resident of the efficient operation of the facility

Education:

  • Licensed and currently registered as a Practical Nurse in New York State.

Minimum Experience:

  • A minimum of one (1) years’ experience is preferred.
  • Demonstrates competence, and experience in nursing skill within the scope of practice

Licensed/Registration/Certification or Permit required:

  • New York State CPR Certification
