Job Summary: We are seeking a dedicated and empathetic Primary Care Provider (PCP) to join our team on a contract basis through DirectShifts. The ideal candidate will be passionate about preventative care, skilled in building patient relationships, and willing to undergo specialized training in Alzheimer's Disease provided by our team. You will play a crucial role in interpreting assessment results and delivering personalized care plans through telehealth consultations.

Key Responsibilities:

  • Conduct telehealth visits with patients to review digital cognitive assessments and blood biomarker results.
  • Build and maintain strong, empathetic relationships with patients and their families.
  • Provide clear and compassionate explanations of screening results and potential next steps.
  • Develop personalized care plans focused on preventative measures and early intervention for Alzheimer's Disease.
  • Collaborate with our team to stay updated on the latest training and best practices in Alzheimer's Disease care.
  • Document patient interactions and care plans accurately in the digital health records system.
  • Participate in ongoing training and professional development sessions provided by the company.

Qualifications:

  • Doctor of Medicine (MD)
  • Board-certified in Family Medicine, Internal Medicine, or related field.
  • Active multi-state medical license (particularly seeking licenses in Texas, Florida, Colorado, and California)
  • Minimum of 3 years of experience in primary care or a related field.
  • Strong interest in preventative care and managing chronic conditions.
  • Excellent communication and interpersonal skills.
  • Proficient in using telehealth platforms and electronic health records.
  • Ability to work independently and as part of a multidisciplinary team.
  • Previous experience or interest in geriatric care and Alzheimer's Disease is a plus.
