Travel Position | Registered Nurse - Case Management | Lonetree, CO
Position Summary:
The RN Case Manager is responsible for coordinating and managing patient care across multiple healthcare settings, ensuring safe, efficient, and quality care. This role involves collaboration with interdisciplinary teams, patient advocacy, and utilization management to optimize clinical outcomes.
Responsibilities:
- Assess, plan, implement, and evaluate patient care needs throughout hospitalization and post-discharge.
- Develop and coordinate individualized care plans based on patient needs, diagnosis, and treatment goals.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care.
- Conduct patient and family education regarding disease management, medications, and discharge planning.
- Facilitate referrals to community resources, home health, and outpatient services as needed.
- Monitor patient progress, track outcomes, and adjust care plans as necessary.
- Ensure compliance with regulatory standards, documentation requirements, and organizational policies.
- Participate in quality improvement initiatives and case review meetings.
- Support utilization management by reviewing and coordinating necessary medical services.
Requirements:
- Active Registered Nurse (RN) license in Colorado.
- Minimum 2 years of case management experience in acute care, hospital, or managed care settings.
- Strong knowledge of medical terminology, disease processes, and healthcare delivery systems.
- Excellent assessment, critical thinking, and decision-making skills.
- Effective communication and interpersonal skills for patient, family, and interdisciplinary team interactions.
- Familiarity with electronic medical records (EMR) and care management software.
- Certification in Case Management (CCM) preferred but not required.
- Ability to adapt to a travel assignment with flexibility in scheduling and assignments.
