URGENT LPN/LVN Need in NYC
Responsibilities
•Schedule: Variable Days: 10:00 AM to 8:00 PM alternating weekends
•Engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare system
•Throughout the treatment process, maintain patient medical records and documentation consistent with state regulations and policy through our HealthPoint and Athena EMR system
•Act as a conduit for the medical treatment of the On-Demand patient population
•Navigate conversations related to management of health, well-being, medication management, and goals of care
•Provide supervised, directed treatment of common medical diagnoses
•Provide care with cultural sensitivity and compassion
•Lead your practice with a patient-centered approach
Qualifications
•Minimum of two years of experience as a Licensed Practical Nurse
•Willing to show proof of COVID vaccination
•Current unrestricted license NYS LPN
