Job Details:
Duration: Full-Time
Shift: 3*8 day shift
Responsibilities:
- Conducts health assessments focused on wound, ostomy, and continence related care. Performs focused physical assessment, develops plan of care based on the diagnosis and individual client needs. Recommends treatments as appropriate
- Assist with the development, revisions, and evaluations of standards of care
- Assess and document patient response to wound care, ostomy, continence, fistula and complex wounds treatment. Evaluate the quality and delivery of care provided by the clinical staff
- Works collaboratively with the health care team and makes referrals to other disciplines as indicated
- Routinely providing comprehensive assessments of patients at risk for developing pressure injuries. Initiating preventative measures.
Required Skills & Experience:
- Valid Registered Nurse (RN) license in the state of Illinois
- Strong communication skills and the ability to work collaboratively within a multidisciplinary team
- Compassionate and empathetic approach to patient care
- 2 years of experience in healthcare preferred
