Responsibilities:

  • Conducts health assessments focused on wound, ostomy, and continence related care. Performs focused physical assessment, develops plan of care based on the diagnosis and individual client needs. Recommends treatments as appropriate
  • Assist with the development, revisions, and evaluations of standards of care
  • Assess and document patient response to wound care, ostomy, continence, fistula and complex wounds treatment. Evaluate the quality and delivery of care provided by the clinical staff
  • Works collaboratively with the health care team and makes referrals to other disciplines as indicated
  • Teach, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems
  • Routinely providing comprehensive assessments of patients at risk for developing pressure injuries. Initiating preventative measures.

Required Skills & Experience:

  • BSN degree
  • 2 years of experience in healthcare prefered 
  • Wound and Ostomy Certification is REQUIRED or 6 years of solid Wound Care experience.
  • Proficiency in utilizing BHMC’s electronic record systems, equipment, and other relevant healthcare and billing systems
  • Good interpersonal skills.
