Job Description
We are seeking a skilled and compassionate Registered Nurse (RN) specializing in Wound, Ostomy, and Continence Care to join our team. The RN will conduct focused health assessments, develop individualized care plans, and provide expert clinical support to promote healing and quality outcomes.
Responsibilities
- Perform comprehensive health and wound assessments; develop and implement evidence-based care plans
- Recommend appropriate medications, treatments, and preventive measures.
- Evaluate and document patient progress in wound, ostomy, continence, and fistula management.
- Collaborate with interdisciplinary teams to ensure coordinated and effective patient care.
- Educate patients, families, and staff on wound prevention and management strategies.
- Participate in staff development, orientation, and continuing education programs.
- Support quality improvement initiatives and best practice implementation.
- Maintain patient confidentiality and uphold professional and ethical standards.
- Manage wound care equipment and ensure proper utilization of specialty resources.

Qualifications
Required:
- Associate Degree in Nursing (ASN/ADN) from an accredited program.
- Active New York State RN License (verified through NYSED).
- AHA Basic Life Support (BLS) certification.
- Minimum 2 years of Med/Surg experience.
- Experience in wound and colostomy care.
- Current certification in one of the following:
- Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB
- Certified Wound Ostomy Nurse (CWON) – WOCNCB
- Certified Wound Care Nurse (CWCN) – WOCNCB
- Certified Wound Specialist (CWS) – ABWM
- Wound Care Certified (WCC) – NAWC
- Bachelor’s Degree in Nursing (BSN).
- Additional wound care or leadership experience.
